Title: Home Care Registered Nurse Case Manager

Location: New Milford, CT

Compensation & Benefits: $95,000 to $125,000 + Comprehensive Benefits Package including Medical, Life, Disability, Malpractice Coverage, CME Allowance, Tuition Reimbursement, 33–38 Days of PTO, and 6 Paid Holidays

Key Responsibilities for Home Care Registered Nurse Case Manager:

  • Deliver in-home nursing care, typically managing 4–5 patient visits per day
  • Perform clinical procedures such as wound vacuum management, pleural drain care, INR testing, dressing changes, and diabetic education
  • Conduct initial evaluations for patients transitioning from hospital care and reassess ongoing clinical needs
  • Design and execute personalized care plans tailored to each patient
  • Accurately document all care and patient interactions within the EMR system
  • Educate and communicate effectively with patients and their families regarding treatment plans and health management
  • Partner with an interdisciplinary home care team and participate in case discussions as needed
  • Provide high-quality, compassionate care to adult and geriatric patients with a variety of medical conditions

Qualifications for Home Care Registered Nurse Case Manager:

  • Active Registered Nurse (RN) license in Connecticut
  • Graduate of an accredited Associate Degree Nursing (ADN) program (BSN a plus)
  • Current BLS certification
  • Valid Connecticut driver’s license and reliable transportation
  • At least 1 year of RN experience
  • Previous home care experience is a plus, but not required
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work autonomously while delivering exceptional patient care in a home setting

Pay: $95,000.00 - $125,000.00 per year
